Very hard if you want to plat the game a lot faster. Survival is the way to go though for pure pissin your pants, gut wrenching, hardcore enjoyment of the game. If you've played Skyrim, you already know most of the mechanics in the game. Start on Hard or Very Hard, and create a separate character for Survival.
